Family & Women’s Health at Avalon Park accepts most major forms of health insurance. However, we recommend that you contact your provider directly to ensure that Family & Women’s Health at Avalon Park is contracted with them, as provider relationships can change, affecting co-pays and coverage.

Please bring your health insurance card, along with your identification, with you so that we can make a copy for your patient record. Anytime that you have a change in insurance, address or phone number, be sure to notify our medical staff during check in, so that the change can be entered into our system.

Filing Insurance Claims

A member of Family & Women’s Health at Avalon Park, located in East Orlando, medical team will gladly file a health insurance claim if the plan is one that our practice currently accepts. It is very likely that any PPO plans that are considered out of network will be filed at a higher out-of-pocket rate. Any additional balances remaining after filing the insurance claim will be due upon completion of services (an invoice from Family & Women’s Health at Avalon Park or Florida Hospital Medical Group will be sent directly to the responsible party once the insurance provider covers their amount).
