Appointments

The medical team at Family & Women’s Health at Avalon Park is accepting new patients, she accepts all ages. If you’re interested in starting a lasting relationship with a medical team located in the Avalon Park community of East Orlando, you should contact us.

Morning, evening and same-day-appointments can be made by calling our practice directly at 407.306.0982. Alternatively, you can fill out the Request for an Appointment form on this website. Please bring your current insurance card, referral form if required, a list of any prescriptions currently taking or refills needed and a list of any questions you may have. If you would like to ask us any questions before your appointment, please fill out a Contact Form to help assist you during your visit.

Billing and Financial Policy

If you have insurance, a member of the Family & Women’s Health at Avalon Park team will file the claim and bill your provider. However, patients are responsible for any required co-payments or outstanding balances. Payments are expected at the time of visit. Family & Women’s Health at Avalon Park accepts cash and credit cards (VISA and MASTERCARD).

Cancellation Policy

Please call Family & Women’s Health at Avalon Park at least 24 hours in advance to cancel appointments, failure to do so could result in a fee of $25.00.

Emergencies and On Call

If you have an emergency, please call the office number at 407.306.0982, which will be redirected to the answering service after hours. You can expect a call back within 30 minutes. If you cannot wait for a return call or have a medical emergency that requires immediate assistance, please go to the nearest Florida Hospital Emergency Department (or any emergency department). Family & Women’s Health at Avalon Park is located on Avalon Park West Boulevard, conveniently situated between SR 408 (East-West Expressway) and SR 528 (Beachline).

Lab and Test Results

As a general rule, Gregorie Constant-Peter, MD, FAAFP will discuss patient lab and test results at a follow-up Family & Women’s Health at Avalon Park appointment. Upon completion of the specific test, further instructions will be given as to how and when the results will be made available. For example, the results of a Pap Test might be sent via a postcard, or annual lab work and routine screenings might be sent back to the practice. It’s best to ask if you are confused about how to obtain your test results.

Late Policy

If you are going to be more than 10 minutes late for your Family & Women’s Health at Avalon Park appointment, we request that you call our office. If the schedule allows, the appointment time will simply be shifted to accommodate the delay.

Prescription Refills

Calls for prescriptions received within the Family & Women’s Health at Avalon Park office after 12:30 pm will be addressed the following day after 2:00 pm. We have 48 hours to process refill requests, if received after 12 pm on Friday the refill may not be processed until after 2 pm on the following Monday. We kindly ask patients to please plan ahead and request there refill at least 7 days prior to their prescription running out to avoid any missed medication doses.

Records Release

Please complete the records release form (Health Information Disclosure Form) to obtain your medical records from your previous family physician, gynecologist and any specialists you may be consulting. Our Practice Privacy Policy is available at the office for your review.

Referrals

You should call Family & Women’s Health at Avalon Park at least three (3) days before the appointment to give us the information needed to complete the referral. That information includes: your name, the physician you are being referred to, his/her phone and fax numbers, the date of the appointment, the diagnosis for the referral, your insurance, whether it is for a visit or a specific procedure (such as surgery) and where it is going to be performed (office or hospital). We do not mail referrals to specialists. You should familiarize yourself with your insurance plan, and determine if it requires you to take a referral with you.
